What kind of documentation do I need to be eligible for Disability Services?

Documentation identifying disability must be from a credible source such as a physician, psychologist, or professional trained in the specific area of disability. This can include relevant reports, assessments and documentation from a diagnosing physician or psychologist and additional information from a current therapist. The documentation needs to be based on current information; old documentation may not be sufficient. It is the student’s responsibility to acquire new documentation if none exists, or if what is provided is not adequate for determining eligibility.
