The Job

A MIHT serves patients in out-of-hospital settings by providing 24/7 needs-based, at-home, integrated acute care, chronic care, and preventative services.

This medical care delivery system emphasizes:

  • Patient-centered navigation
  • Transparent population-specific care by integrating existing infrastructure and resources
  • Defining operations through population-based needs assessment and tools
  • Bringing care to patients through technology, communications, and health information exchange
  • Leveraging multiple strategic partnerships operating under physician medical oversight
  • Improved access to care and health equity through 24-hour care availability by utilizing local EMS resources
  • Delivering evidence-based practice using multidisciplinary and interprofessional teams in which providers utilize the full scope of their individual practices, supporting healthcare delivery innovation.

The MIHT supports and augments, rather than replaces, other healthcare delivery models by providing an optimized mix of care, with costs and readmissions lower than traditional out-of-hospital care models.

The Program

Gain an understanding of critical care through 6 weeks of live, instructor-led online classroom learning, followed by 16-24 hours of on-site skills training, and then a 100-hour clinical practicum with a primary care, home health, or community paramedic provider.

This program will focus on providing high-quality community-based education with complementary skills training for the MIHT, in partnership with Kaiser Permanente of Oregon.

After successful completion of the program, students will be prepared to take the Board for Critical Care Transport Paramedic – Community Paramedic.

Prerequisite: Paramedic with at least 2 years of full-time urban EMS experience, or 4,000 hours.

In-Person
  • Module 7: 4 days of on-site skills training and evaluation
  • Module 8: 100-hour primary, urgent, and home health care internship
Certifications

Students will be prepared to take the Board for Critical Care – Community Paramedic exam following successful completion of this training.

In addition, PCC-IHP has applied for institutional accreditation from the Continuing Education Coordinating Board for Emergency Medical Services to provide CECBEMS credits for the MIHT and other continuing education programs.

Who should enroll?

Paramedics with at least 2 years of full-time urban EMS experience, or 4,000 hours.

The Investment

  • Time: 6 weeks of online classroom learning, followed by 16-24 hours of on-site skills training, and then a 100-hour clinical practicum with a primary care, home health, or community paramedic provider.
  • Tuition: $2,490, plus textbooks and the cost of travel, lodging, and food for on-site skills training at the PCC Institute for Health Professionals.
