This form is designed for PCC employees to request adds, moves, and changes to printers, phones, computers and faxes. A service desk "ticket" will be created based on your responses to the form.
This process can take up to 5 business days – please let us know if your request is urgent.
For other types of service requests please use the IT Service Request form.
Please create a spreadsheet and attach it below. On the spreadsheet, provide the following information for each item to be moved:
- Nature of request – phone change, extension change, or voice mail change
- Names of employees
- Email addresses of employees
- Current PCC extensions of employees
- Change requested – please be clear and specific
- Required date of completion
