This form is designed for employees to request assistance from IT (Information Technology) at PCC. When you answer the questions below, a service desk "ticket" will be automatically created.
- Urgent requests (classroom down, business-critical service i.e. Banner, network, phone outage, etc.): Dial 971-722-4400.
- Classroom software requests: Use the Classroom software request form.
- Printer support requests: Include the printer name, printer model, and if it has a Panther Print sticker (all found on front of printer).
- Phone or network-related requests: Use the Network/Phone Request form.
Short (50 characters) version. You will have a chance to explain further in the next question.
If you are experiencing an issue with your computer, be sure to include the brand of device (Mac or Dell), along with your location (on-site/off-site). For requests that need manager's approval include manager's email address.
